WebPlus X6: Using X6 for shopping cart form

I'm trying to figure out how to create an add to shopping cart link that adds a customized item to a cart for checkout.

Customer will need to select the main item then have multiple sub categories for which they can select how many, if any, they would like to add.

I tried this with the buy now form on serif as well as sending a form to a shopping cart, but have not had luck.
