WebPlus X6: Form Submission Acknowledgement Page

I have created an acknowledgement page which tells users that their form has been submitted. By editing the form properties I have redirected it to the acknowledgement page. However, when I preview the site, and complete the form and subsequent captcha, I'm getting a 404 Not Found Error.

Please advise as to what I can do to connect it to my acknowledgement page.
